Being informed and familiar with healthcare providers is important. To ensure your OAD experience is positive, we provide Patient Information links that allow you to access insurance plan/network participation, OAD Medical Forms, and various educational resources.

You can discover what health insurance plans/networks OAD's physicians are contracted with via the "Billing & Insurance" link in the left-hand column. We strongly recommend, however, that patients contact their health insurance carrier/network Member Services Representative prior to scheduling an OAD appointment to confirm benefits and coverage.

During OAD's appointment scheduling process, our Scheduling Department will obtain some registration information and may mention the need to complete Medical forms for your appointment.

In addition, we make available some of our practice brochures for your review and offer several "Links of Interest" that connect you to insightful and informational websites.

OAD Orthopaedics is now part of Cadence Physician Group. All future communications from OAD moving forward will be from Cadence Physician Group.

For services beginning November 1, 2012 you will see statements from Cadence Physician Group. You will also start to see our new name on the explanation of benefits from your insurance company. Please make your checks payable to the name indicated on the statement you are paying. For questions on billing, please call the number listed on the statement you receive.
